What I do is if the head wont come off by tugging it, ill grab the lego neck with my teeth and pull. (were you holding the neck, or the head with your teeth)
neck.not enough traction on the head...
WOW! this head must REALLY be stuck! 
there are actuly 2 heads like this. one is Han Solo with storm trooper armor, and the other is a Death Star Trooper
