What are your thoughts on my Outro template below? Do you think it is good enough? Suggestions?

Other than that, I think it looks good!
I like the design, but there is a bit too much extra text that clutters things up. I say remove the text below the Subscribe, G+, and Twitter icons, as most people will know what those do. Then you could shorten the text under the Patreon to something like "Help support my videos by donating on Patreon!", or something.
I'd also suggest removing statue-you, and just filling up the space with the previous video, to make it more prominent.

Far too crowded and garish. There is way too much text about the place; you certainly don't need a description for straightforward things like Subscribe, Google+ and Twitter, nor do you need such a lengthy explanation of the Patreon bit. I say cut the whole 'Top Patrons' all together, as are you really going to go back and update it every time your top Patrons change?
Scrap the lower half entirely; get rid of what I assume is you (nice body you have there) and the Patron thing, and replace it with two previews of your other videos. Finally, flush the yellow boxes down the toilet. Get rid of them completely. Replace the yellow boxes with a subtly coloured background. Choose a minimal colour that will draw attention away from itself and instead to the 'Subscribe' and social network buttons + your other video links.
Also, if possible, change the twitter logo to the current one, opposed to the ancient one you currently have. It always irritates me and activates my OCD .

Better? I just really think having the TOP Patrons setup to the right is necessary in order to reward future patrons.
Keep in mind that this will be modified before imported into the final cut of each of my videos.

I have to agree. By not using an outro template, it creates a more professional and polished air. Honestly, if a viewer wants to find more of your work or a social networking account, they'll look it up on your profile.
My advice would be to just leave your films as they are. Or if you still want to link things, I would put that in the video description.
I agree w/ Squid & Eddie. I'd rather have my video without these things begging for subs. If people like your material, they'll follow you. Adding links to the description, as Eddie said, is the way to go.
Yeah, I'm not going to add anything much, but I do have to agree with these guys here. But, I do wish you some good luck with whatever it is you're intending to do, Lucas.
