I have repeatedly tried to update my avatar, but it just keeps showing the old one, it is the right size, image type, and has a different name than the old one.

It's your cache -- the forum database stores it in the same location. Clear your cache or refresh the page a couple of times and it should be fixed.
I had the same problem I shall try that now smeagol.
